Referral form/lab requisitions/kit draw forms need to be filled out completely and emailed or faxed to MMP along with a copy of insurance card (front/back) 48 hours prior to scheduled appointment. This helps us insure we have proper equipment and prepare for your specimen collection. Follow your provider instructions (fasting or drink plenty of water.)
After staff has arrived to perform the blood draw/specimen collection, your specimen will be delivered to the designated laboratory for specimen processing. Specimens are processed under the protocol of the laboratories policy. Results are sent directly to you and/or your ordering provider.
Accepted Payments
MobiMed Plus, LLC is an independent phlebotomy/lab service. We do not bill insurance companies for our services, any insurance information collected is forwarded to the testing laboratory for billing of test(s). MMP accepts the following forms of payment for specimen collection:
